How can I get a copy of my GED Certificate?

I was awarded a GED at Everett Community College in the mid 90's, and I need to know how I can get a copy of it if possible. Or at least the date it was awarded? My full name is David Patrick Hayes

Once you complete your GED test, you will receive an official GED certificate in the mail within one month from the date your final scores were posted. To order additional copies of your certificate, you must submit a GED Certificate Request form to the Oregon Department of Community Colleges and Workforce Development.
