When you're growing tomatoes, "green shoulders" means when the top of the fruit (by the stem) doesn't ripen. It stays green or maybe turns yellow, but never turns red. During the growing season this happens when the tomatoes get too hot during the day. This also happens at the end of the season when parts of the plant start to die off -- there's less protection from the sun because there are fewer leaves.
